Request for Qualifications for Special Inspection Agency

Eastern Kentucky PRIDE, Inc., is seeking Statements of Qualification for Special Inspection testing and inspection services as defined in Chapter 17 of the 2013 Kentucky Building Code for the building structures (main building, entry pavilion, and restroom building), earthwork below building structures, and fireproofing is included. Quality Control for site work (including, but not limited to over excavation and engineered backfill of soil undercut outside the building pad, drainage subgrade course, drainage wall backfill to walls, stone base course for pavements, site concrete walks and pavements, site retaining walls and their foundations, and site pavements) where specified to be by Owner within the construction drawings and specifications is also to be included in this scope of work. The related work would be in relation to the proposed Appalachian Wildlife Center, located in the former industrial park in Bell County approximately 10 miles southeast of Pineville. The Owner will accept Statements of Qualifications from firms interested in working on this project until Friday, October 12, 2018, at 2 p.m. ET. The Statement of Qualifications will be considered by a selection committee appointed by the Owner. The Owner reserves the right to reject any and all submittals. See full RFQ package for details. No electronic or faxed responses will be accepted. Late responses will be returned to supplier unopened. Attention of respondents to this RFQ is called to the requirements as to conditions of employment to be observed under the contract, Section 3 of the Housing and Urban Development Act of 1968; Section 109 of the Housing and Community Development Act of 1974; Title VI of the Civil Rights Act of 1964; Executive Order 11246; Certification of Non-segregated Facilities; and all other applicable federal and state laws and regulations.
